The Appointment Process
How to Book Appointment
- Booking an appointment is easy! Just call us at 804-655-0893 or send us a message and request an appointment for a consultation.
- Once your appointment is booked, we will send you a link for the required Intake Form which includes a clinical questionnaire and the consent forms.
- Please return the completed Intake Form at least 48 hours prior to your appointment, otherwise we will need to reschedule your appointment. Signing the consent forms is not a commitment to treatment. Signing only acknowledges that you have received the information within the consent form.
- Once we receive the completed paperwork, we can order labs for you if necessary.
- The cost for labs can be paid at time of consultation by cash, credit card, or debit card.
Please note...
- Payment for services will be due on date of service. Cash, debit card, credit card, and HSA are acceptable forms of payment.
- **Please arrive at your appointment on time. Late arrivees may be asked to reschedule. We want to make sure you get the full consultation you are paying for and without interrupting the schedule of the patient after you.
How to Find Our Office
We are located within the Balance RVA office building at 10431 Patterson Ave, Henrico, VA 23238. Check-in at the front desk or on the iPad located at the front desk.
